Strange rule by the Health Ministry, Oman!

Regarding drug control, Oman and Bangladesh are in the opposite direction. In Bangladesh, any doctor can write any medicine. Not a doctor, even a pharmacist can give any medicine to any customer. Not a pharmacist, even any patient can ask for any medicine and she/he will get that including antibiotics. Apparently, almost all the medicine are OTC in Bangladesh!!

In Oman, rules are maintained to some extent. We, the general practitioners can not prescribe all the medicines. Our stamps and signatures are not as powerful as that of a specialist. There is a list of controlled medicines only a specialist can prescribe. For example, anxiolytics and antidepressants (except for one or two), and some steroid ointment can’t be prescribed by us.

Today I got a new circular. For the last 6 years, some of the steroid ointments and nasal sprays that I used to prescribe, are now included in the controlled medicine list. Previously Clobetasone and Betamethasone were on that list only. We could prescribe mometasone, beclomethasone, fluticasone (spray) and hydrocortisone ointment. Today’s circular prevents us to write these simple steroid ointments/creams too! Now onwards, only a dermatologist or a specialist can write these items.

This is not a wise decision I think. We get skin allergy patients invariably. A steroid ointment/cream is required for such a condition. Now we can’t prescribe any!! Moreover, dermatologists or specialists are not readily available in all the clinics. It will only increase the suffering of the patients; they will only get tablets from us or they have to spend more money to see a specialist!

We hope that the authority will change this new rule!

On Thursday, the last working day of this week for me, I consulted 27 patients in my chamber. It wasn’t that busy time in the clinic.

In the evening, after 9 pm, I walked on the treadmill for around 15 minutes. It helped me get 10k steps, Alhamdulillah.

In terms of "more money," apparently, the specialist would profit, but in terms of "more work" and "time consumption" for those actually "unnecessary" patients, they might get oversaturated very soon.

Assuming the specialists probably wouldn't want to give up on the extra money coming to them this way, most likely, they would hold back from addressing any complaints to those in charge, and instead, they would start creating long waiting lists.

All of it would also surely raise the dissatisfaction of the people/patients due to unnecessary extra costs and, afterward, possible long waits for examination, prescription, etc.

The way we see it, in the long run, it's a lose-lose decision for all the parties involved, apparently made without good, detailed, and throughout thinking.
