what happens to people who can't afford medical aid? What is the government health sector like?

As long as one is a Resident, there are medical aid but from what I read before moving over, I can't get free medical aid if I am not a Resident yet but instances might also vary.
Generally treatment in NZ is fairly and accommodating.

what happens if you're not a resident, but are on a work visa for instance? perhaps working toward becoming a resident (and what does that entail?)?

I am on work visa, critical skill, tier 2. There are several ways to working towards resident by earning points.
It could be earning points via, higher wages, more work experience in NZ, Level 3 or above certification etc.

There are options to becoming resident, not easy but also not unreasonable difficult. I actually prefer their system here compared to my 15 years stay in South Africa that earned me no residence.
